Diagnosis and Procedure Codes on Paper Claims

Where submission of paper claims is necessary, we ask that you use the international diagnosis and procedure codes when completing the claim forms, rather than the description of the diagnosis and service. For your reference, Guardian Life uses International Classification of Diseases, 9th revision (ICD 9) and Current Procedural Terminology (CPT) codes, which are available online. The use of the international codes will maintain patient confidentiality and reduce processing time for claims which will result in faster reimbursement.
