Claim reimbursements of $5M and greater will be sent by Electronic Funds Transfers (EFT) only. If you are likely to receive payments greater than this amount, it is strongly suggested that you complete applicable Provider Claim Settlement Form or Member Claim EFT Settlement Form and submit to our office.
