Requirements for Broker Proposals

- Name of Company
- Demographics for persons to be covered
- Type of Company (i.e. Security Firm)
- Previous carrier, if any
- Current Utilization report (at least 12 months)
- Current benefits to which experience applies
- Breakdown of coverage units (ideally, at the beginning and end of the experience period)
